Health Information and Forms
California law requires that all students attending public and private schools be properly immunized in order to protect the health and safety of students, staff, and the broader community. Under California Education Code section 48216, schools must exclude students who do not meet state-mandated immunization requirements unless a valid exemption applies, as defined in the California Health and Safety Code. Parents and guardians are responsible for providing documentation showing that their child is up to date on required vaccinations or qualifies for an allowable exemption before their child may attend school.
