Steps to Enroll in the Kinder C.D.C. Program
  1. In March enroll at your elementary home school. If you do not know your home school you can look it up through my school locator at http://locator.decisioninsite.com/?StudyID=162052

  2. Request from your school site office administrative assistant a CDC kindergarten enrollment request form. The staff will initiate the application and send the parent/guardian the link to a request from. 

  3. Submit the signed form to the main CDC via the online platform as possible, in the month of March.

  4. When you turn in the signed form we will give you an enrollment packet for the CDC kindergarten program.

  5. Return the completed CDC kindergarten enrollment packet and a $150.00 non-refundable registration fee to the main CDC office no later than the last business day in March.

  • School of Choice - If you intend on requesting a School-of-Choice application, CDC will not be able to save a spot at the requested school. We will only hold a space at your home school. (exception for Dual Language and TK programs)
